A Social Return on Investment Analysis of Improving the Management of Chronic Obstructive Pulmonary Disease Within the Spanish National Healthcare System

Abstract:

The aim is to define a set of proposals to improve the current management of chronic obstructive pulmonary disease (COPD) in the National Health System (NHS) from a comprehensive and multidisciplinary perspective, and to evaluate the impact of their implementation from clinical, healthcare, economic and social perspectives.

A group of 20 COPD-related stakeholders (healthcare professionals, patients and informal caregivers, among others) participated in an online Delphi process to reach a consensus on a set of 15 proposals to improve the current management of COPD within the NHS in four areas of analysis: diagnosis, risk stratification, management of the patient with exacerbated COPD and management of the patient with stable COPD.

A 1-year social return on investment (SROI) analysis was performed to estimate the impact of implementing the set of proposals relative to the investment required. A sensitivity analysis was used to test the robustness of the model.

The hypothetical implementation of the set of 15 proposals would require an investment of 668 million euros and would generate a social impact of 2,079 million euros in terms of savings for the NHS and improvements in the quality of life of patients and their informal caregivers, among others. Thus, for each euro invested in the set of proposals, a social return of 3.11 euros would be generated (2.71 euros in the worst scenario and 3.62 euros in the best scenario), both tangible (32.6%) and intangible (67.4%).

In conclusion, the implementation of this set of 15 proposals would generate a positive social impact that would triple the necessary investment. The results can serve as a basis for decision making regarding health policy and practice in relation to the management of COPD in the NHS, thus contributing to reducing the large burden of COPD.
